What is Lockout Tagout (LOTO)?

Lockout tagout is a procedure that ensures equipment cannot be started while someone is working on it. Every energy source feeding it is disconnected, a lock is fitted to the isolating device and a tag is attached carrying the name of whoever placed it and the date — and only that person removes their own lock. The governing rule: one lock per person, one key in their hand.

Why it matters

The most dangerous moment in maintenance is not the work itself but unexpected start-up during it: someone else restoring power, a control system resuming automatically, or stored energy releasing suddenly. Lockout tagout exists to make that physically impossible rather than merely agreed verbally.

The energy most often forgotten is not electrical but residual: pressure trapped in a line, a suspended weight that can drop, a compressed spring, retained heat, or fluid that can flow in from another direction. This is why isolation does not end at opening a breaker — it ends at verifying that the energy is actually at zero.

Examples of what gets isolated

  • Primary and secondary electrical supply to the equipment.
  • Compressed air or hydraulic lines feeding it.
  • Fluid or gas supply valves, secured in the closed position.
  • Moving parts that could travel under their own weight, mechanically supported.
  • Control systems that could restart automatically when power returns.

03The procedure

How it is applied — six steps

  1. Notify

    Inform everyone affected before the equipment is shut down.

  2. Shut down

    Stop the equipment through the correct operating sequence, not by abrupt cut-off.

  3. Isolate

    Disconnect every feeding energy source — electrical, pneumatic, hydraulic, fluid and gas.

  4. Lock and tag

    A lock on each isolating device, and a tag carrying the name and date. Every member of the crew applies their own lock.

  5. Release stored energy

    Bleed pressure, support suspended parts, release springs, allow heat to dissipate.

  6. Verify zero

    Attempt to start the equipment through the normal control to confirm it does not respond, then return the control to off.

That last step is the most frequently skipped and the most important. Verification is what turns isolation from an assumption into a proven fact.

In a Saudi context

On sites where maintenance is carried out by contractor crews, a recurring challenge appears: overlapping lock ownership. Equipment is isolated by one crew and worked on by another in a later shift, leaving responsibility for lock removal unclear at handover. This is why a record of who applied each lock and when, together with documented shift handover, is part of the procedure rather than administrative overhead on top of it.
